Lost or Stolen Registration

Lost, stolen, or damaged registration documents can be replaced by any registered owner on record

• An Affidavit of Loss form can be completed in advance with a notarized signature or the form can be completed in our office witnessing the signature. 
• The fee of $14.50 will replace the registration reflecting all previous up to date information.
• The registration will reflect the current plate and tab number.
